Who we work with

Every setting has its own hurdles to getting people care. Here's how we help yours.

Hospitals and health systems

Discharge shouldn't stall on finding follow-up care. We schedule quickly, so the people you discharge leave knowing where their care continues.

Primary care practices

Mental health needs often show up first in your exam room. Refer to one team for therapy, medication support, and addiction recovery, and stay updated on every referral.

Therapy and counseling practices

When a client needs medication support, you shouldn't have to hand them off. We add medication management alongside your work, so you stay their therapist.

IOPs and PHPs

Progress can slip in the gap after a program ends. We help people step down to ongoing outpatient care that keeps their momentum going.

Addiction recovery programs

Keep recovery on track after your program ends. We provide outpatient medication-assisted treatment, including buprenorphine, alongside therapy.

Schools and community organizations

You see the need every day, but not always a clear place to send people. We'll build a referral path around your community, with a community liaison as your direct contact.

How referring works

  1. 1

    Send a referral

Use the Referral Portal, our referral form, or your community liaison.

  2. 2

    We reach out

Our team contacts the person you referred by phone and email in less than 24 hours.

  3. 3

    They get scheduled

Often for a same- or next-day appointment, at a time that works for them.

  4. 4

    You stay in the loop

We keep you updated. The portal adds daily status and one-click progress notes.

Sindanny P.

For a while, I didn't think I would find a place where my mental health would be taken seriously. Thankfully, I was referred to Mindful Care. I'm very happy with my treatment & the approach my provider has to help me with my needs. HIGHLY RECOMMEND!

Christopher D.

My provider is pleasant, respectful and shows concern. I am very happy with her treatment and medication management. I have had trust issues with other providers through different platforms but with my provider, I feel comfortable and relaxed to be open about my mental health and trust her diagnosis and recommendations. With out her help, I would have gone untreated.

Evelyn C.

My therapist is great. She's so intuitive and she understands me very well. She calls things about me that hit it right on the nose. I'm glad to have her. Most people think therapists are enablers but not her. You have to be honest and be comfortable with being vulnerable with her and she can help you. I recommend her to anyone who's really looking to change their life.

Brandon K.

Frequently asked questions

Partnering with us

How do I become a referral partner?

There's no agreement or sign-up needed. You can refer to us today through the Referral Portal, our referral form, or your community liaison. Your liaison can also meet with your team for a short in-service on how we work together.

Who is the right fit for Mindful Care?

We provide outpatient care for people with a wide range of mental health needs, from everyday stress to complex, ongoing conditions. We're not the right fit for someone who needs inpatient, residential, or emergency care. Our providers also do not prescribe long-acting injectables, habit-forming medications, or controlled substances, with two exceptions: buprenorphine in our addiction recovery program, and stimulants when a clinician deems them appropriate in our ADHD program. If someone needs a higher level of care, we'll help connect them with trusted partners.

Do you accept insurance?

Yes. We're in-network with 100+ insurance plans, including Medicaid, Medicare, and TRICARE. We accept Medicaid in all six states we serve.

Is there a cost to refer?

No. Referring to Mindful Care, including using the Referral Portal, is free for our partners.

Sending a referral

What information do I need to send a referral?

You'll need the person's email address, phone number, date of birth, and state of residence, plus their current care setting and the service you're referring them for. You can also add any other details or forms you think will help.

Do I need the person's consent before I refer them?

No consent is needed to send a referral. A signed HIPAA authorization is required before we can collaborate on care or share progress notes.

What happens right after I send a referral?

Our team reaches out to the person you referred by phone and email in less than 24 hours to get them scheduled. When you refer through the Referral Portal, you'll also get a confirmation email, and your community liaison is notified.

Can I refer someone for a specific service?

Yes. When you send a referral, you can note the service you think fits best, and our clinical team will confirm fit during intake. See what we offer in your state.

What are the ways I can refer?

You can refer through the Referral Portal, our referral form, or your community liaison. You can also email our referrals team at referrals@mindful.care. Only referrals sent through the portal can be tracked in your portal account.

What if someone needs care urgently?

We reach out to every person referred in less than 24 hours, and many are scheduled for the same or next day. When a referral can't wait, check the urgent box in the Referral Portal so our care management team knows, or contact your community liaison directly. If someone is in immediate danger, call 911 or the 988 Suicide & Crisis Lifeline.

Can I refer someone who is in crisis?

If someone is in immediate danger, call 911 or the 988 Suicide & Crisis Lifeline first. Mindful Care provides outpatient care and is not a substitute for emergency services.

Referral Portal

What is the Referral Portal?

The Referral Portal is a free, HIPAA-compliant way to send referrals and follow each one, with daily status updates like Patient Contacted, Patient Scheduled, and First Appointment Complete. Each person on your team creates their own account in under a minute. Learn about the Referral Portal.

How do I get progress notes?

After a first appointment, click “Request Progress Notes” in your dashboard. With a signed HIPAA authorization, our care management team emails them to you within 5 business days.
